Naftopidil Powder also called naftopil, naftopiil, chemically named 1-(3,4-dimethoxybenzyl)-2-(1-naphthylethyl)piperidine hydrochloride, is a drug with multiple pharmacological effects, mainly used to treat peripheral vascular disease and lower urinary tract symptoms caused by prostatic hyperplasia. It is a new type of long-acting, highly selective α1-receptor blocker. By selectively acting on α1A and α1D receptors, it can relieve the sympathetic tension of the prostate and urethra caused by the excitation of the receptors, reduce urethral pressure, increase urine flow rate, improve or eliminate urine reflux in the prostate and ejaculatory ducts, and help dredge the prostate gland ducts and eliminate the residual secretions of pathogens in the prostate gland ducts, thereby achieving the therapeutic purpose of improving and eliminating symptoms.

α₁-Adrenoceptor Antagonism and Subtype Selectivity

Naftopidil antagonizes α₁-adrenoceptors, reducing the corresponding adrenergic responses. Its receptor profile shows a relative preference for α₁D rather than exclusive activity at this subtype.

An original study using cloned human receptors reported the following affinity relationships:

Comparison - Reported relative binding affinity

α₁D versus α₁A:Approximately 3-fold higher affinity for α₁D

α₁D versus α₁B:Approximately 17-fold higher affinity for α₁D

Lower Urinary Tract Smooth Muscle and Tissue Function

product-1536-1024

Naftopidil can be used to investigate adrenergically stimulated responses in the prostate, urethra, and related tissues.

An original study compared prostatic pressure and blood-pressure responses in dogs and combined these experiments with cloned human receptor-binding assays. The findings supported investigation of how α₁A and α₁D affinity contributes to tissue-level functional selectivity.

Reduced smooth-muscle tone does not establish a reduction in prostate size. Acute relaxation and long-term tissue growth are distinct endpoints requiring separate assessment.

Isolated-tissue experiments can combine norepinephrine or phenylephrine stimulation with nonadrenergic contraction controls.

Bladder Afferent Signaling and Neural Regulation

Naftopidil research extends beyond urethral smooth muscle to bladder storage reflexes and sensory afferent pathways.

In a rat model of cerebral infarction-associated detrusor overactivity, naftopidil increased bladder capacity. This effect disappeared following resiniferatoxin treatment used to desensitize C-fiber afferents. The investigators proposed involvement of C-fiber-associated pathways in the lumbosacral spinal cord.

These findings represent functional evidence from a specific animal model. They do not independently establish that naftopidil selectively blocks a particular C-fiber ion channel.

A spinal-slice electrophysiology study also reported that 100 μM naftopidil reduced Aδ- and C-fiber-evoked excitatory postsynaptic currents. Because this experiment used a high concentration, interpretation should consider noncanonical effects and comparability with achievable tissue exposure.

Cell-Cycle Regulation and Exploratory Antiproliferative Research

product-1536-1024

Beyond its established receptor pharmacology, naftopidil has been investigated for effects on cell proliferation and cell-cycle regulation.

An original prostate cancer study reported growth inhibition accompanied by G₁-phase arrest:

  • In LNCaP cells, p21 and p27 expression increased.
  • In PC-3 cells, p21 increased without a corresponding increase in p27.
  • Growth inhibition was also reported in a PC-3 xenograft model in nude mice.

Stereoselective Metabolism and Pharmacokinetic Research

Naftopidil contains a chiral center, making it useful for investigating differences in metabolism between enantiomers.

A human in vitro metabolism study identified contributions from multiple CYP enzymes. CYP2C9 and CYP2C19 inhibitors strongly inhibited R(+)-naftopidil metabolism, while inhibitors of several other CYP enzymes produced varying degrees of inhibition.

Consequently, its metabolism should not be reduced to a universal single-enzyme description.
